🌡️ REGENERATIVE MEDICINE WITH PRP 🔹PRP or Platelet Rich Plasma is a blood-derived product that consists in a plasma suspension containing more platelets than the normal blood. It also contains leukocytes, red blood cells and plasma proteins. 🔹Preparing PRP starts with a good desinfection of a vein to take a sterile blood sample. This blood is centrifuged in a special syringe to obtain the plasma : the principle behind PRP generation is the separation of liquid and solid whole blood components. This platelet rich plasma is separated from the rest of the blood by gravity because platelets are smaller and less dense than leukocytes and red blood cells. 🔹Platelets are a vital source of growth factors and proteins, all of which are released during the reparative processes occurring in the early phases of tissue healing. They help to optimize the healing in bones, wounds, ligaments and tendons. 🔹This product is also used in joints for the treatment of osteoarthrosis. This horse was presented with OCD in the hock. One of the most frequent orthopedic pathologies in horses. Even small OCD fragments (see video) can cause joint irritation and provoque ‘wear lines’ on the cartilage. If these fragments are not removed in an early stage, they can cause degenerative joint disease and arthrosis after some years. 👉🏻 OCD is a developmental disease that can affect all joints and is expressed and manifested by the formation of intra- articular fragments (OCD) or irregularity of the subchondral bone and cartilage OC). 👉🏻Treatment of OCD is relatively straightforward and if done in time (before damage to the articular cartilage) the prognosis for the horse is excellent in most cases. 👉🏻Up to 30% of the yearlings are affected with OCD. All breeds are susceptible to it, but the disease develops less in ponies and other slow growing breeds. 👉🏻The etiology of OCD is multiple and can be divided into genetic and environmental factors. ✔️Our advice: 👉🏻If you have a foal, a radiographic check-up at 18 months of age is recommended. If the foal shows joint swelling or lameness before this age, consult your veterinarian immediately. 👉🏻If you have a horse with OCD and you are not sure if the fragments need to be removed or not, you can always ask the advice of our Orthopedic Surgeons who have an enormous experience in this field. 🔵What is an inguinal hernia? The phenomenon occurs when a part of the intestine (usually a small part of the small intestine) slips trough the inguinal ring into the scrotal sac. ⚠️The danger of this situation lays in the vascularisation problems of both the intestine and testicle. The blood flow in both organs is compromised and they start to die off gradually. At this point, the problem is called a strangulated inguinal hernia and needs specialized treatment asap! 🔵 When does it occur and how can you recognize it? Some stallions are born with large inguinal rings. The are more prone to develop an inguinal hernia, typically after work or after reproduction activity. The horse are typically painful (scratching, lying down, rolling) and the affected testicle is pulled upward, is harder and can feel cold(er). Is this situation the horse should be moved to a surgical center as fast as possible. 🔵What to do? In very acute cases the intestine can sometimes be removed from the scrotal area via transrectal palpation, however this is relatively unusual. If the intestine is still stuck after 20-25 min of massage, surgical intervention is unavoidable (inguinal and sometimes abdominal). If the intestine is too damaged, abdominal surgery is required to remove the necrotic intestine. If the testicle is too damaged, (unilateral) castration is necessary. 🔵 Can we prevent? The size of the inguinal ring is easily palpated trough transrectal examination. If the inguinal ring is too large, it can be reduced surgically by minimally invasive laparoscopic intervention. It is an excellent and proven way to prevent inguinal herniation, avoiding the risk of anesthesia. If a horse already was subject to inguinal herniation, it is always advised to perform this intervention. #horse #horseriding #pferd #paarden

◾️ Standing arthroscopy of the carpus 👉🏻 This horse was referred to our surgery department for a diagnostic arthroscopy of the intercarpal joint. Pathology in the joint and subchondral area where identified on MRI and CT examinations. Diagnostic arthroscopy was the next step to determine the extent of damage and to make a treatment plan.
