A male contraceptive that reportedly will last around 13 years could be available to the public in the next seven months, according to scientists in India. The Indian Council of Medical Research completed clinical trials for the contraceptive, which has been sent to the Drug Controller General of Indian for approval. The contraceptive is a replacement for surgical vasectomy and loses potency after about 13 years. “The product is ready, with only regulatory approvals pending with the Drugs Controller. The trials are over, including extended, phase 3 clinical trials for which 303 candidates were recruited with 97.3% success rate and no reported side-effects. Rare case of foreign body - this patient came to the ER with a yankee candle lodged in his rectum!! Entrapped anorectal foreign bodies are being encountered more frequently in clinical practice. Many patient say they simply slipped and fell on the object. Majority of cases are the result of insertion during erotic activity. Most objects are introduced through anus; however, sometimes, a foreign body is swallowed, passes thruogh the gastrointestinal tract, and is held up in the rectum. Numerous objects, including billy clubs, various fruits and vegetables, light bulbs, bottle, Impulse body spray cans, and turkey basters have been described as retained rectal foreign bodies. There is a wide variety of objects and variation in trauma caused to local tissues of the rectum and distal colon. One of the most common problems encountered in the management of rectal foreign bodies is the delay in presentation, as many patients are embarrassed and reluctant to seek medical care. Most of these patients present to the emergency room after efforts to remove the object at home. Moreover, in the emergency room, patients may often be less than truthful regarding the reason for their visit, leading to extensive workups and further delays. Even after extraction, delayed perforation of or significant bleeding from the rectum may occur. Hence, a stepwise approach that includes diagnosis, removal and postextraction evaluation is essential. Typical clinical findings of a rectal foreign body are pain with defecation, abdominal pain, rectal bleeding, pruritus, and constipation. Delays in seeking medical attention and multiple attempts at self-removal can lead to mucosal edema and muscular spasms, making removal of the object more difficult. Diagnosis is confirmed by plain abdominal radiographs and rectal examination. Manual extraction without anaesthesia is only possible for very low-lying objects. Patients with high- lying foreign bodies generally require general anaesthesia to achieve complete relaxation of the anal sphincter to facilitate extraction. @medicaltalks

Extreme case of hydrocephalus in a pediatric patient!! Hydrocephalus is a pathology characterized by dilation of the cerebral ventricles, normally caused by the obstruction of the flow of cerebrospinal fluid (CSF), or by an imbalance between its production and absorption, in addition there is inflammatory response, neuronal damage and destruction of periventricular axons. Hydrocephalus is cataloged in two ways. Depending on the origin, it can be congenital or acquired; and in relation to the CSF flow, this pathology is classified as communicating (i.e. CSF can exit the ventricular system, but the fluid is not absorbed readily, or there is too much fluid to be absorbed) or non-communicating (i.e. CSF cannot exit the ventricular system, and thus there is obstruction to CSF absorption, resulting in an increased pressure inside the brain). Symptoms of hydrocephalus may include an unusually large head with thin, transparent scalp, bulging forehead with increased spaces between the bones of the skull (fontanelles), and a downward gaze. The most common congenital cause is the obstruction or narrowing (stenosis) of the cerebral aqueduct — the long, narrow passageway between the third and fourth ventricles of the brain. It can also be caused by infections, hemorrhage or a tumor. The condition is pretty rare and can be easily detected early in pregnancy by ultrasound which does not necessarily shows the obstruction but detects an abnormal collection of fluid. This is oral candidiasis, also called oral thrush, which is caused by a fungal infections involving the skin and mucous membranes. The causative agent is Candida albicans, which actually is an asymptotic inhabitant in half of the healthy adults. In healthy individuals, Candida exists harmlessly in mucus membranes such as your ears, eyes, gastrointestinal tract, mouth, nose, reproductive organs, sinuses, skin, stool and vagina, etc. It is known as your “beneficial flora” and has a useful purpose in the body. When an imbalance in the normal flora occurs, it causes an overgrowth of Candida albicans which may lead to an opportunistic infection. The mucosal surfaces primarily affected by candidiasis are the oral cavity, esophagus, angles of the mouth, and genitals. Overgrowth of Candida is protected against by local T cells and interleukin-17 Thus, when immunity is compromised, growth proceeds unchecked and leads to infection, notably in patients who are on immunosuppressive therapy, infected by HIV, or have diabetes.
