A frequent ❓ that I get when lecturing is about repairing or adding to margins of porcelain veneers or porcelain crowns. 🦷 In this case I demonstrate step-by-step the technique I use to repair a failing margin on an existing porcelain veneer. Firstly, you have to determine which type of porcelain was used to create the veneers. 🕵️ I used a microfill composite as the surface material due to its incredible polishability, and its ability to maintain its polish long term. In fact, the microfill should have the same luster as the porcelain restoration after polishing. A limitation of microfill composites: They are not radio-opaque. 🙅‍♂️ In some cases, I use a microhybrid or a nanofill composite below the microfill to make the restoration radio-opaque. Typically, I will use a microhybrid flowable composite as a first subsurface layer. After curing the flowable composite, the microfill composite is layered over the surface of the flowable composite, and onto the porcelain veneer. After polymerization, the composite is blended to the porcelain using fine diamond burs (8392-016 Brasseler), composite polishing discs (Flexidiscs, Cosmedent) and rubber polishing cups (Flexicups, Cosmedent). The composite polishing is completed using an aluminum oxide polishing paste (Enamelize, Cosmedent) with a Flexibuff polishing disc. So here's our problem: Even though we have hybrid and nanocomposites that are more opaque than microfills, sometimes they're just not opaque enough. 🤷‍♂️ Often just a touch 👌of opaquer can transform a restoration, blocking out a dark zone, or blocking out the translucency of the composite. See, if we add more dentin composite to block out, we might run out of room. That is, there may not be enough space for the enamel composite because we had to use such a thick layer for the dentin composite. 👉So the advantage of opaquers is that they can block out in very thin layers. ☝️So why not just use opaquers all the time instead of using dentin composites? The problem with opaquers is that if you put them on too thick, the restoration will look too opaque, almost like a bad PFM Crown. It is really the art of balancing using your dentin composites with opaquers so that you can create as natural a restoration as possible. 🦷✨ Opaquers are used to: 1️⃣block out underlying stain 2️⃣eliminate shine through of less opaque composite 3️⃣also in our class fours to help blend composite to the tooth structure. Generally speaking, my opaquers are going to be used on the dentin composites to continue the blackout, but under the enamel surface. Have you ever had trouble balancing your composites and opaquers? 🦷Patients that present for restorative treatment who also have root exposure from gingival recession can create challenges in the perceived esthetics of the treatment. 🦷 This patient was referred to me for restorative treatment after implant failure 🙅‍♂️ in the attempt to replace the missing teeth #23 and #24. The patient opted for a traditional crown and bridge approach for tooth replacement with teeth #22, #25, #26 and #27 prepared as abutments for the zirconia bridge. A zirconia bridge with natural root shape and color was tried in and evaluated for patient approval. I was 🤯by the ceramist’s ability to match the root color and shape seamlessly with her natural dentition – rarely have I seen this executed so well. The patient, however, declined this treatment option stating that “the bridge made her teeth look old” and asked that the root areas be covered with pink porcelain to make the teeth look more “natural and esthetic” (her words). The bridge was returned to the laboratory to have the root areas of the bridge covered with pink porcelain. 💅 Photographs were taken for soft tissue matching and the bridge was returned with pink porcelain covering the root areas of the pontics and abutments. The bridge was bonded to place using the APC protocol as defined by Prof. Marcus Blatz. 👉 What do you think? 👉 Which option would you choose? 👉 Should I have told the patient that she was wrong in electing the pink porcelain instead of the natural root colorization? Here are 3️⃣ things I would do differently if I had this case again: 🕗 Spend more time in the temporary phase, first doing root staining on the provisional and finding out if the patient accepted that. 🦷 The preparations on teeth #25 and #26 should have been shorter and rounder. ☝️Avoid having sharp or thin incisal edges on your preparations – the milling will leave a void between the incisal portion of your prep and the intaglio of the restoration.
