Well, this is terrifying for anyone who gets fillers! This is a 54-year-old, healthy female who presented with this firm red lesion under her eye. She has had a history of facial fillers under her eyes and on her cheeks for the past 5 years. A biopsy of the lesion was taken and she was diagnosed with xanthomatous filler reaction associated with delayed hypersensitivity. This is the body responding to the injected foreign material sometime after it is injected (not right away). When the filler is injected, it can migrate which may take the foreign body reaction longer to occur.  In this case, she was treated with medications to dissolve the fillers, steroids, and antibiotics and her lesions have gradually resolved.

Rosai–Dorfman disease (sinus histiocytosis) is a rare cause of lymphadenopathy (enlarged lymph nodes) in children under 10. Usually, when kids present with this, they have swollen lymph nodes in their neck that go away spontaneously. Most cases involve the lymph nodes in the neck, but it can be seen in other lymph nodes throughout the body. About 40% of the cases can be extranodal (outside of the lymph nodes). In this case, this is a 4-year-old male who presented with swollen lymph nodes on both sides of his neck over the course of 3 months. For the past 3 weeks, he was having difficulty breathing while lying down. PET/CT showed multiple enlarged lymph nodes throughout his body, with the largest being in his neck. A biopsy was taken to rule out an infection or malignancy like lymphoma. Under the microscope, there were multiple cells called histiocytes present in the lymph nodes. Histiocytes are a type of white blood cell that is present in the tissues called macrophages that are responsible for ingesting foreign substances. He was treated with steroids and it resolved over 3 months. He has been symptom-free for over 1 year. The cause of Rosai-Dorfman disease is unknown but it is thought to be a response to a possible infective agent.
