Essa semana tive o prazer de encerrar um ciclo de 3 aulas, onde discutimos a otimização terapêutica dos pacientes com Dislipidemia. Back in the saddle again... Sitting still for the next few days, I guess I’ll take the time to explain what’s happening. Sigh. The last couple days my legs have been extremely swollen, along with my face as time went on, usually signs of something going on with my heart; but I figured I was just dehydrated, or ate one too many tacos with my corn dog. Yesterday, along with the swelling, pain started in my right chest, which I’m usually used to the left side when I had my previous complications. After an hour of painful breathing, I felt prompted to text my surgeon/cardiologist with my symptoms, who gave me three serious options of what it could be, telling me to go to the hospital for testing. After 20 minutes in the ER, my EKG’s & tests were so bad they were treating me for heart attack. They life-flighted me to IMC (The ‘Death Star mothership’ of cardiology) when I could’ve just taken an Uber, where they found not a heart attack but a new left-bundle-branch-block, my heart functioning at 30% efficiency (worse than before my open heart surgery) diagnosing me with Heart Failure. (Stage D Heart Failure) No amount of attempted calm breathing stopped my tears & I was afraid to fall asleep. I am at the moment being stabilized with medications, as there is discussion/high possibility of a pacemaker that I’m hoping to avoid. I’m scared, frustrated, goofy-tired, all the feels. All I can do is try to stay positive, and look at the slick hospital floors wishing I had my roller skates... I am grateful for Chris and his care for me. Again. I am grateful my girls are grown this time, keeping busy doing their thing in Moab for the summer. I’m grateful for friends, family & neighbors who have sent their love today in many ways, collectively filling and healing my damaged heart; it’ll be a long haul & I appreciate you all so much. Since our childhood our parents stopped us and made us sit for a while when we sneezed and wanted to go out right? Why was that? Is there any proper reason why? Well many beautiful answers came in, the best answer was that " when we sneeze our heart stops for a second!" But if it had been true, all of would have had a huge Cardio problem, because when our heart stops for a second, our body cannot sustain such a shock!. But what actually happens when we sneeze? The answer is here Dr. Richard Conti, past president of the American College of Cardiology answered the question about sneezing and the myth about the stopping heart! So where did the myth originate that your heart stops when you sneeze? The changing pressure in your chest due to sneezing also changes your blood flow, which may change the rhythm of your heartbeat. Dr. Richard Conti, past president of the American College of Cardiology, speculates that the belief that the heart actually comes to a stop during a sneeze could result from the sensation of having the heart “skip a beat.” When there is a prolonged delay before the heart’s next beat, he said, that beat is then more forceful and more noticeable, perhaps as a funny sensation in the throat or upper chest (Ray, 1992). So our ancestors asked us to wait till our body comes back to normality after a sneeze & we are here still calling it a superstitious belief! But when someone in the house sneezed, we were not allowed to go out! Isn't a superstition? Well when someone is not feeling that well it is not manners to go out! Worse outcomes in women have been described following both conservative and invasive treatments of CAD. For example, increased levels of residual platelet reactivity during treatment with antiplatelet drugs, higher rates of adverse cardiovascular outcomes following percutaneous coronary revascularization, and higher operative and long-term mortality after coronary bypass surgery have been reported in women compared with in men. Despite the growing recognition of sex-specific determinants of outcomes, representation of women in clinical studies remains low and sex-specific management strategies are generally not provided in guidelines. Personal growth is so cool. When I first started medical school, I was so overwhelmed that I would drink like 5 espresso shots and stay up all night studying before exams. I’d take my test feeling jittery, unfocused and would immediately crash afterwards feeling nauseous and burned out. Into second year (& with the help of my therapist), I accepted that I’m too old for that mess & learned to force myself to get AT LEAST 4 hours of sleep... & I performed much better. Yesterday, I took my final exam as a 3rd year student. Not only did I get all the sleep I needed, but I was able to wake up and do a sunrise hike & meditation on the mountain. (IM STILL SHOOK). When I got to school, one of my classmates asked me why I’d even consider working out before such an important test, to which I said, “There’s never a good time... If we expect our patients to do it through their life stresses, we should too.” Think about it. . . . To all the pre-meds & underclassman, keep pushing. You will figure things out. Que dia! Hoje participei da cerimônia anual de convocação de novos fellows do American College of Cardiology e finalmente fui empossada Fellow do ACC! Uma honra enorme para cardiologistas americanos e, ainda maior para brasileiros que, como eu, raramente recebem essa honraria de uma das principais sociedades de cardiologia do mundo! Ao final de meu pós-doc em Harvard a Dra Lynne Stevenson e o Dr Peter Libby (dois dos principais nomes da cardiologia mundial) me indicaram para fellow do ACC. Em 2019 recebi a notícia de que minha nomeação havia sido aceita. Em 2020 seria se cerimônia de posse, que acabou não ocorrendo em função da pandemia. Hoje,dois anos depois, finalmente tive a oportunidade de participar dessa cerimônia emocionante! É claro que passa um filme na cabeça, é claro que o caminho não foi fácil, e teria sido mais difícil sem o apoio de tantas pessoas importantes e queridas (amigos, familiares, mentores, pacientes, alunos). FIRST IN-PERSON CONFERENCE PRESENTATION ☑️ (and first in-person conference as a resident!)🫀 • this weekend was so busy and a complete whirlwind. working as many hours as you do as a resident is exhausting. trying to do research on top of residency hours is not at all easy and often times I come home and it feels like a hassle because I’m just very tired. spending this last week in the CCU, getting poster together, figuring out travel logistics, and the busy schedule of events at the conference itself has left me needing a solid 3 days of sleep! • despite how exhausted I am, this weekend was filled me with so much energy and inspiration. I had such a great time attending lectures on topics I’m passionate about, hearing lectures from leaders in the field that I look up to, going to panel discussions on a variety of topics, networking, meeting some previously virtual mentors IRL, meeting my ACC women in cardiology cohort, got to spend time with my fellow cardiology-bound co-residents and our Temple mentors outside of work, support my TempleGang as they showed off their own research, and presented my own first in-person poster! • also a surprise twist — ran into a handful of my @stgeorgesu classmates & networked with some cardiologists who earned their degrees from SGU! who says Caribbean grads can’t make it in competitive fields!? • #americancollegeofcardiology #ACC #ACC22 #WIC #womenincardiology #preventivecardiology #youarewhatyoueat #fitness #diversifycardiology #thefaceofcardiology

I absolutely love what I do! Can’t begin to tell how many people told me I shouldn’t become a cardiologist-That it’s “too hard” for a woman and I’m “too nice” to be one or that the “perils of the cath lab are too much for a woman”. I heard these statements and sometimes held on to them. But then I would always fall back on my passion. Durante 17 anos, pesquisadores americanos coletaram dados sobre a dieta de 3003 pacientes. Modelos multivariáveis foram ajustados por idade, sexo, escolaridade, consumo de álcool, fumo e atividade física. Alimentos ultraprocessados fornecem 58% da energia total na dieta nos Estados Unidos. Em média, os participantes consumiram 7,5 porções por dia deste tipo de alimento. Cada porção diária adicional foi associada a um aumento aproximado de 10% no risco de doença cardíaca grave. Last week the ACC (American College of Cardiology) guidelines on the primary prevention of heart disease were updated. But something is very different this time around❤️ * “The document emphasizes healthy lifestyle as ‘the most important part of prevention throughout the entire lifespan, but these guidelines also look at prevention from a NEW lens of social determinants of health’.” #medscape * Based on scientific evidence, we now know that only 10% to 20% of our health is actually determined by the healthcare that we receive, and 70% to 80% is impacted by social determinants of health🏃🏻‍♀️ * Underserved and low-income populations have a higher risk of heart disease and life expectancy can vary by MORE than 20 years in people living only 5 miles apart. This is ASTOUNDING.
